Awake Abdominoplasty/Tummy Tuck (Avelar Technique)

The abdominoplasty procedure involves the removal of excess skin by the surgeon from the middle and lower abdomen while in some occasions, tightening the abdominal muscles. Stretch marks that are located on the excess skin can also be taken out at the time of removal. The surgeon makes an incision stretching from hip to hip hidden in the underwear or bikini area. The location of the incision is above the pubic area. Another incision is made around the belly button.

Introduced in the 1990's by Brazilian surgeon Juarez M. Avelar, MD, this procedure uses a smaller incision than the standard abdominoplasty (Tummy Tuck).  It combines abdominoplasty with tumescent liposuction.

Juarez Avelar, MD, suggested that in a standard abdominoplasty, the undermining of the abdominal flap which consisted in the rupture of the lymphatic and perforator blood supply caused wound complications. To reduce these complications, he developed a new surgical technique that avoids wide undermining, which he presented at the 36th Brazilian Congress of Plastic Surgery in 1999. This technique was later then modified to include the use of tumescent anesthesia.

Here at our office, the Avelar technique is done with liposuction under local tumescent anesthesia.  The fat in the abdomen area is removed while keeping blood vessels intact. The fat removal is less traumatic as well as the surgery to remove excess skin.
Men and women who have loose, hanging skin, and with excessive fat and/or a protruding belly are candidates for a an Avelar Abdominoplasty. The ideal candidates are patients in relatively good physical shape, who are unhappy with an excess of skin or fat in the abdominal area that is resistant to exercise.
Often women who have been through pregnancy and childbirth have a laxity of underlying muscles which makes it very difficult for mothers to achieve a flat and firm tummy again. This is because the skin and muscles have stretched beyond the point where they can naturally return to their normal location and shape.
Other ideal candidates for abdominoplasty procedures are those who have lost a significant amount of weight and have excess sagging skin in the abdominal area.

Older patients, who may have lost skin elasticity or have sagging skin due to the ageing process, are also good candidates for this procedure.
